How Can I Notify You of a Change of Address?
You may either call our toll free number at: 800-579-2878, e-mail us at: LSWService@lifeofsouthwest.com, or download, print, complete and mail in the Change of Address (form number 9075) to the address at the top of the form. It is important that LSW be notified as soon as possible of a change of address.

How do I Notify You of a Name Change?
You may either call our toll free number at: 800-579-2878, e-mail us at: LSWService@lifeofsouthwest.com, or download, print, complete and mail in the Policy Change Request (form number 5277) to the address at the top of the form. It is important that LSW be notified as soon as possible of the change. If your name has changed due to marriage or divorce don't forget to update your Beneficiary information.

How do I Notify You of a Beneficiary Change?
You may either call our toll free number at: 800-579-2878, e-mail us at: LSWService@lifeofsouthwest.com, or download, print, complete and mail in the Change of Beneficiary (form number 5026) to the address at the top of the form. It is important that LSW be notified as soon as possible of the change.

How Often do I Receive an Annuity Statement of Account?
Your statement frequency is dependent on the type of policy and activity of payments on your policy. All annuity policies (not in a payout status) receive at least an annual statement.

How do I Get a Duplicate Policy?
You may either call our toll free number at: 800-579-2878, e-mail us at: LSWService@lifeofsouthwest.com, or download, print, complete and mail in the Request a Duplicate/Certificate of Your Policy (form number 4007) to the address at the top of the form. As noted on the form, if the policy type is an annuity and over one-year-old, the cost of a duplicate is $25.00. A Certificate of Insurance is available at no cost. Certificates of Insurance are available for all other policies.
